To ensure a fair and thorough evaluation, we ask that you provide specific examples, success stories, and/or measurable impacts wherever possible. Rather than simply stating that policies or programs exist, we encourage you to highlight real-life cases, outcomes, and creative efforts that demonstrate your organization’s commitment to Diversity, Equity, Inclusion, Belonging, and Accessibility (DEIBA). Be as specific as possible!

This assessment will be scored on a 1-10 scale, with 1 indicating minimal effort or impact and 10 representing outstanding, innovative, and far-reaching contributions to workplace inclusion.
